Good day and welcome to this week’s episode of the Nursing Show podcast for nurses, by nurses. I’m your host Jamie Davis and I’d like to take this opportunity to thank you for checking out the show this week. First up, in this week’s nursing news items, we’ll look at how specially trained nurse navigators are helping mom’s get through their pregnancy and early days after the birth of their babies.

Then in this week’s special tip segment, we’ll take a look at an initiative from the American Academy of Ambulatory Care Nursing and the Medical-Surgical Nursing Certification Board to create a new nursing certification in patient care coordination and transition management. We’ll chat with Marianne Sherman, president of the AAACN about that program later in the show. That and more is coming up later on in this episode.

Tip of the Week – Care Coordination and Transition Management Certification for Nurses

ShermanRNs are uniquely qualified to perform care coordination and transition management, or CCTM, in all settings. To better prepare RNs for a CCTM role in which they must manage patients with complex health care needs from an access, quality, and cost perspective, American Academy of Ambulatory Care Nursing (AAACN) conducted a two-year expert panel translational research process. The result was the creation of evidence-based CCTM core competencies, a core curriculum text, and 13-module online course, which is scheduled for release this August.

\The CCTM exam is the next step in this process, bringing together the quality of Medical-Surgical Nursing Certification Board’s certification exam development and administration with the CCTM content expertise of AAACN. I got the chance to chat with AAACN President Marianne Sherman about the new certification and how it will change the way we help patients transition from the hospital to the next stage of their care.
